Single Session Clinic

The Single Session Clinic is an opportunity to meet and talk with a Registered Social Worker about life stress or changes in your life. Specific issues that you need  to find a next step for.  It may be a specific problem, or just looking for advice. This service is open to all ages.
 

The single session is offered on a drop-in basis for up to 45 minutes in length. Patients who present more than twice in a row will be asked to speak to their physician for a referral to the Mental Health program.


These single sessions are being offered at both Grand Ave. and 20 Emma St. locations.


Single Session clinics are offered weekly, on Wednesdays, from 9 a.m. to noon. Patients need to arrive before 10:45AM.

Take the FHT Challenge!

The Thamesview Family Health Team is challenging patients to take control of their health and lead healthier lives. By taking a “preventative approach”, we are encouraging healthy lifestyle habits in patients with risk factors for Heart Disease and Diabetes. This program will help those take control of their health BEFORE problems start, or while they are in the beginning and reversible stages of various health issues. The program will include topics such as:

- Hypertension
- Diabetes/Pre-Diabetes
- High Cholesterol
- Healthy Weights
- Diet
- Unhealthy Habits
- Physical Activity
- Stress

Upon successful completion of the 2 group sessions, each Patient will receive an individual 30-60 minute assessment/counselling from a FHT Nurse to discuss their specific problems and goals. The dietitian may be brought in for additional diet counselling for the patient as well.
The patients will have follow up appointments to track their progress (i.e., monthly, bi-monthly, etc.) for a period of 6 months after which they will be discharged from the program.

Suggested criteria to qualify for the program are:
Waist Circ.: Men > 102 cm
Waist Circ.: Women > 88 cm
Triglyceride level < 1.7 mmol/L (mod. risk) and < 1.5 mmol/L (high risk)
HDL-C: < 5.0 mmol/L (mod. risk) and < 4.0 mmol/L (high risk)
Blood Pressure > 140/90 mmHg or 130/80 mmHg (CKD or DM)
Fasting glucose level > 5.6 mmol/L or diabetic
BMI: >25

Education Sessions will be offered on:


Mondays 10 a.m.-12:00 p.m. (Emma)
Wednesdays 2-4:00 p.m. (Grand)
Thursdays 6-8:00 p.m. (Grand)
