FAQs

What is a Family Health Team?
A Family Health Team represents a collaborative approach to primary health care, uniting a variety of health care providers to coordinate the best possible care for you—the patient.
What is primary care?
Primary health care is your initial point of contact with the health care system, whether it be a doctor, nurse practitioner, or another member of the healthcare team. It encompasses disease prevention and management, acute care, and health promotion.

Will I always see my own Family Physician?
While we strive for you to see your primary provider, you might be seen by a registered nurse, nurse practitioner, or a resident family doctor depending on the nature of your appointment. As a teaching site for Western University, we provide diverse learning opportunities for new family doctors. Rest assured, all care providers are highly trained to offer you the best possible care.
How soon should I inform my doctor or primary pharmacy about medication refills?
Please notify your pharmacy at least 2 weeks in advance when you require medication refills. The refill process involves multiple steps and coordination among several individuals.
How do I update or obtain a new Health Card?
For a lost, stolen, or expired health card, visit the Service Ontario website for guidance on replacement. For more information, the Health Care Ontario website can provide additional assistance.
