Aging Well is a comprehensive program led by our occupational therapist for patients, focusing on the physical body, the mind, and safety.

Services include:

  • Memory Assessments
  • Dementia Screen
  • Fit to Drive Assessment
  • Cognitive Therapy
  • Community Resources
  • Mobility and Balance Assessments
  • Osteoporosis / Postural Assessments
  • Conditioning / Exercise Programs
  • Home Safety Assessments
  • Prescription of Assistive Devices – for the home
  • Mobility Assessment and Funding (e.g. canes, walkers, wheelchairs)
  • Concussion Management
